For a bureaucratic machine, a person — it is a collection of documents and certificates, those, as Mikhail Zhvanetsky rightly put it, “mute witnesses of a long road”, without which one cannot take a single step. But they are our best lawyers, helping in everyday life situations, from allowing access to the pool to closing absenteeism at work.
The range of medical certificates is huge, as are the types of their forms, and the expiration date. However, all of them are valid only if filled out correctly. And this already requires special care from the filling and — special vigilance from recipients.
Medical certificates: strictly according to the form, with all seals
Mistake #1. Grammar. Unfortunate mistakes in names and surnames made when filling out certificates can create a situation similar to the one in which the hero of Karpenko-Kary's play "Martin Borulya" found himself. Alas, the letter “e” that crept into the surname instead of "o" was the reason for the refusal to confer on him a title of nobility. And although in everyday life we are talking about solving more prosaic issues, nevertheless, this can serve as a serious reason for the accounting department in case of refusal to pay.
- The rules for filling out medical certificates do not allow errors or any corrections.
Error #2. Form not specified. For exemption from physical education, one form of certificate is required, for visiting the pool — another, and for rest in the sanatorium — third. If there was a short illness, then the doctor writes out a certificate of the appropriate form, and in case of a long illness — other.
- In the upper right corner of the certificate, the form in which it was issued should be indicated.
Error #3: All required seals are missing. The validity of the certificate is confirmed by three seals.
- Must be stamped by the medical institution where it was discharged. It must contain the details, profile and location of the hospital.
- Second, round, seal is placed by the attending physician.
- The third, triangular, is confirmed at the registry.

Mistake #4. All dates are missing.
The form must have two dates. The first, with which temporary disability began, and the second, indicating when a person will be able to return to work.
Error No. 5. The certificate was issued by the "rear" number or issued by another institution.
A certificate can only be issued on the day of contacting a doctor and issued in an institution entitled to medical activities, which must be confirmed by proper licenses. Otherwise, it is not valid.
