The causes of hyperandrogenism in women can be different, the main ones are genetic factors, disorders of the hypothalamic-pituitary system, ovarian, adrenal dysfunction.

And if there are reliable laboratory criteria and tests for the diagnosis of central, adrenal and ovarian forms of hyperandrogenism, then there are a number of difficulties associated with the complexity, cost of existing methods, as well as limited the number of criteria defined in this study.

This indicates a further need to study the mechanisms of interaction of sex hormones and their metabolites in women who have acne, which will contribute to a deeper understanding of the pathogenesis of the disease, improve methods for diagnosing dermatosis and develop a rational, pathogeneticski reasonable method of therapy.

Study of the hormonal background of women with acne

In the course of a study on the study of hormonal levels in women with acne, 108 women aged 17 to 35 years were examined. All patients were subject to a comprehensive clinical and laboratory examination, which was prescribed at the time of diagnosis and at the next dispensary observation. The control group consisted of 25 healthy women aged 18-30.

Among the methods used to determine hormonal status, the following were used:

  • radioimmune − determination of the content of dehydroepiandrosterone sulfate (DHA-S), total testosterone, sex-binding globulin;
  • immunoenzymatic − determination of the content of follicle-stimulating hormone, luteinizing hormone, prostate specific antigen (PSA);
  • statistical − assessment of reliability and determination of correlations between the studied indicators.

When analyzing clinical manifestations of acne at the time of examination, it was determined:

  • mild dermatosis severity − in 11.1% of women;
  • moderate severity − 67.6%;
  • severe course of the disease − in 21.3% of patients.

Analysis of the hormonal status probable changes in the levels of FSH and DHA-C in the blood serum of women with acnewas not detected.

Sex hormone levels and degree of acne

In 17.6% of women with acne, a significant increase in testosterone levels was obtained. According to the results of the correlation analysis , a high direct relationship between the levels of T and LH.

From the anamnesis, 90% of this category of patients revealed:

  • menstrual cycle disorder by type of oligomenorrhea;
  • increased oiliness of the skin;
  • increased number of rashes before menstruation.

In 60 women (56%) of the total number of examined, a significant increase in the level of serum PSA was found compared to the control group, where a deviation was observed in 7% of patients. At the same time, dependence of the degree of increase in this indicator on the clinical manifestations and course of acne in women.

In 45% of patients, an increase in PSA correlated with a decrease in DSH. In 27% of the examined women, an increase in PSA alone was observed with values ​​corresponding to the norm of all the studied values.

The results of the study show that the levels of PSA in the blood serum of women with acne are higher than in the control group and are directly proportional to the severity of the clinical manifestations of the disease.

Furthermore, initial PSA concentrations are directly related to the amount of dry blood, and therefore − free testosterone, which supports the hypothesis that PSA synthesis in women is due to prolonged androgen stimulation.
