Rhinoplasty – This is an operation that helps to make the nose beautiful and neat, and the face more harmonious. And if someone has a congenital nose shape – this is the height of perfection, then for others it can be a serious problem. In order for rhinoplasty to be as effective as possible and provide the desired result, it is necessary to comprehensively evaluate all facial features. Often, patients may be recommended a combination of rhinoplasty and other procedures, which the director of the Nove tilo clinic, candidate of medical sciences, surgeon of the highest category Oleg Timofey told estet-portal.com readers in more detail.
Which plastic surgeries are the most interesting for you?
Early in my career, I favored the simplest of operations. Now, after 18 years of work, my favorite procedure – it is a surgical facelift, which is one of the most difficult operations in plastic surgery. It requires a detailed knowledge of the anatomy of the face, since during a surgical facelift, the doctor has to work in close proximity to the nerves that are responsible for facial expressions. Therefore, when you successfully perform such a complex operation, you feel real satisfaction. Rhinoplasty carries less emotional burden, but requires a jeweler's approach. Fractions of millimeters matter.
Which type of rhinoplasty is more effective: closed or open rhinoplasty?
In my practice at the Nove tilo clinic, I use both open and closed rhinoplasty. When it is necessary to remove the hump on the back of the nose, I am more likely to prefer closed rhinoplasty. But if the patient has a curvature of the entire nose or its tip, then it is better to have an open operation.
The tip of the nose is made entirely of cartilage, and open rhinoplasty will allow them to be better seen and matched.
Open rhinoplasty makes it possible to widely use grafts to correct the nose, or to strengthen the tip of the nose with threads. Therefore, the choice of operation mainly depends on the goals set for the surgeon.
Does the result of rhinoplasty depend on the thickness of the skin?
The most difficult patients to work with are very thick and porous skin. During rhinoplasty, the skeleton of the nose, which consists of cartilage and bones, is reduced. It is necessary to move them in such a way that the skin cover is reduced and takes on a given shape. But if the skin is initially very thick, then it is difficult to contract, and making such a thin nose is quite difficult. At the same time, we cannot perform the removal of excess skin on the back or tip of the nose, as scars will be visible. Sometimes after the operation you have to take long courses of hormone therapy. If, two months after rhinoplasty, the skin has not contracted – glucocorticoids are injected into the tip of the nose, which contribute to the thinning of fibrous tissue. In this way, we gradually get thinner skin and, accordingly, a more elegant nose tip.
What procedures can be combined with rhinoplasty to get the most harmonious result?
Indeed, one cannot consider the nose separately from the whole face. It happens that it only seems that the nose is big and long. But, if you look in more detail, it becomes obvious that a person has some kind of mismatch, for example, between the upper and lower jaw.
Due to the fact that the lower jaw is very short and protrudes little forward, the nose may appear larger.
At the clinic Nove tilo in combination with a reduction rhinoplasty, it is possible to perform a chin augmentation. Sometimes patients may be advised to consult a dentist to correct the bite, or an oral surgeon for surgical correction of the position of the jaw. But, since these are quite aggressive and traumatic operations, patients rarely decide on them. Most often, simpler methods can be dispensed with, for example, a silicone implant can be installed in the patient's chin area. Thanks to this, the nose will appear smaller and the face will look more harmonious. Another operation that can be performed at the same time as rhinoplasty is lip augmentation. It is possible to carry out surgical lip augmentation, namely VY plastic surgery, or non-surgical, by injecting fillers based on hyaluronic acid. I do not recommend combining rhinoplasty and blepharoplasty. Both operations are accompanied by swelling around the eyes, as a result of which patients endure the postoperative period quite hard.
