Face lifting with SaHA fillers: points and injection technique

Number of patients: 6.

Age of patients: 30 – 40 years old.

Comorbidities: none.

Complaints: looking tired.

Diagnosis: bone resorption and redistribution of fat compartments. Agreed Therapy: Calcium hydroxyapatite filler injections (Radiesse) to replenish facial volume using the Bold Lift technique.

Initial levels of tissue deficiency in patients varied. Merz Aesthetic Scale (MAS) was used for evaluation.

The rheological properties of CaHA ensure the precise injection of the drug into problem areas in order to compensate for the lack of skeletal support, smooth out contours, correct sagging in the piriform fossa, flatten the zygomatic zone and atrophy of the upper and lower jaws.

CaHA filler was diluted with lidocaine 2% in a ratio of 5:1 before injection.

The drug was administered supraperiostally as boluses at 7 points (Fig. 1A):

1. On the bend of the eyebrow – 0.3 ml needle 25 gauge, 25 mm;

2. On the lateral part of the eyebrow – 0.2-0.3 ml needle 25 gauge, 25 mm;

3. On the lateral part of the cheekbones – 0.2-0.3 ml needle 25 gauge, 25 mm;

4. On the pear-shaped fossa – 0.2-0.3 ml with 25 gauge needle, 23 mm;

5. On the chin – under the mental muscle 0.2 & ndash; 0.3 ml with a needle 25 gauge, 23 mm;

6,7. On the anteromedial part of the cheek (2 points in the infraorbital foramen) 0.2-0.3 ml with a 25 gauge needlera, 25 mm.

All patients received 3.6 ml of filler.

Results of face lifting with CaHA fillers

Evaluation was performed at rest before and after the procedure using the MAS scale and the International Global Aesthetic Improvement Scale (GAIS).

Standardized photographs were taken before the filler, immediately after the correction, as well as 1 and 3 months after the procedure in profile. The face was photographed at an angle of 45 degrees on the left and right sides, as well as in profile on both sides.

Injection correction of the lower jaw line: important aspects

At the follow-up visit, the degree of improvement in the position of the eyebrows at rest, as well as the degree of improvement according to GAIS (according to the patient and the doctor) were measured. These zones are chosen because there is no validated scale that would be used to quantify the rejuvenation of the entire face.

To get good results, did:

  • one procedure for patients with cheek hollowness initially estimated at 1-2 points on the MAS scale;
  • two procedures 3 months apart for patients with cheek hollowness score of 3–4 points and eyebrow position – at 1–3 points on the MAS scale.

After 3 months after the correction, all patients showed an improvement in the fullness of the upper cheek zone and the position of the eyebrows at rest by at least 1 point on the MAS scale.

Procedure results:

  • visible lifting of the outer corner of the eye;
  • enlargement of the anterior anteromedial projection of the cheeks;
  • more defined contours and a smoother chin surface;
  • instant brow lift;
  • an increase in the projection of the anteromedial part of the cheeks.

Similar degrees of improvement were observed in all 6 patients.

Hyper-diluted CaHA also promotes collagen and elastin synthesis and provides a firming effect.

There were no serious side effects or complications after the procedure. Immediately after correction according to the Bold Lift protocol, patients experienced :

  • moderate swelling;
  • ecchymosis;
  • erythema.

These phenomena, typical of injection procedures, disappeared spontaneously after 2 days.

During the examination of patients 1 and 3 months after the correction no complications were recorded.

The technique Bold Lift creates a scaffold for soft tissue augmentation and contouring. Due to its viscoelasticity CaHA:
